Because of the frequent changes that
occur with medical insurance, it is necessary for us to have you review and
update your personal as well as insurance information at each visit. If you have
a change of name, address, phone or insurance company any time during your care,
please let us know immediately.

New patients to the practice are asked to arrive 15
minutes prior to their appointment for new patient registration. If you like you
may print and fill out the following forms at your leisure prior to your first
appointment. Please remember to bring them with you.

Referrals are the responsibility of the patient.
Please check with your insurance company or your primary care provider if you
think one may be required.

If an emergency arises, and time permits, you may call our office for
assistance. Depending on the severity of the emergency, we may make arrangements
for an immediate office visit or direct you to go to a hospital.
If you need to contact a physician after office
hours, please call our office and follow the prompts to be connected to our answering service. The service will put you in touch with the physician on-call.

LifeTime participates in most insurance and managed care plans. It is
important that you understand our professional services are provided and charged
to you, not your insurance company. Some insurance plans only pay for a portion
of the cost of services, some don't cover at all. Our billing staff will
work with you to verify coverage and benefits. You are responsible for any
balance your insurance does not cover. It is your responsibility to know your
policy and its benefits. With your permission we will prepare and submit your
claim to your insurance carrier. If additional medical information is requested
to process your claim, we will provide it. At each visit, you must provide us
with your current insurance card that we will copy and keep on file. It is your
responsibility to notify this office of any change in insurance providers.
Failure to do so will leave you financially responsible for any services
rendered.

As a service to you, we accept cash, personal checks, American Express, Master Card,
Discover and Visa. All co-pays are due in full at the time of your visit. If there is a balance owed, you will be sent a bill, which is due upon receipt.
