Because of the frequent changes that occur with medical insurance, it is necessary for us to have you review and update your personal as well as insurance information at each visit. If you have a change of name, address, phone or insurance company any time during your care, please let us know immediately. |

New Patients |

New patients to the practice are asked to arrive 15 minutes prior to their appointment for new patient registration. If you like you may print and fill out the following forms at your leisure prior to your first appointment. Please remember to bring them with you. |

Referrals |
||
Referrals are the responsibility of the patient. Please check with your insurance company or your primary care provider if you think one may be required. |

Emergency Services |
||
If an emergency arises, and time permits, you may call our office for assistance. Depending on the severity of the emergency, we may make arrangements for an immediate office visit or direct you to go to a hospital. If you need to contact a physician after office hours, please call our office and follow the prompts to be connected to our answering service. The service will put you in touch with the physician on-call. |

Billing and Payment |

As a service to you, we accept cash, personal checks, American Express, Master Card, Discover and Visa. All co-pays are due in full at the time of your visit. If there is a balance owed, you will be sent a bill, which is due upon receipt. |
